Cardiovascular Patient Education Tools November 2021 The Canadian Cardiovascular Pharmacist Network (CCPN) is an independent group of Canadian pharmacists involved in cardiovascular practice and research. Their mission is to optimize the health outcomes of Canadians with or at risk of cardiovascular disease by promoting excellence in clinical practice and research. They also strive to enhance the knowledge and skills of pharmacists and their colleagues through the advancement of the pharmacotherapy of cardiovascular diseases. CCPN advocates for the optimal use of cardiovascular pharmacotherapy and one of the ways in which this is achieved is through educational initiatives. One important CCPN initiative has been the development of patient and provider education tools. Looking for Peer Support Program and Services for Patients In Canada September 2021 The Canadian Women's Heart Health Alliance (CWHHA) is looking to identify current virtual and in-person peer support programs and services being offered across Canada, both within the healthcare systems and the broader community. The CWHHA's goal is to find out how women are connecting with others, particularly women, after a heart disease diagnosis or heart event so it can better facilitate access to existing supports that are right for them. If you are aware of a peer support program or service being offered in Canada for women living with heart disease, please take a moment to complete the questionnaire below. A Call to Action to Broadly Implement Transformative Community Programs - A statement from the Hypertension Advisory Committee

Recommendation: Broadly and systematically implement established evidence-based community programs that foster healthy living and enhance the prevention, case findings, diagnosis, treatment and control of hypertension.
Purpose: This statement supports the advancement of a Pan-Canadian Framework for Healthy Blood Pressure that advocates for strengthening community action through the better coordination, integration, and utilization of existing programs and resources.
